I have tried the reset button and plugged a phone directly into the modem without any luck. The power and DS lights are lit solid green, but the US button is blinking green.

Let's try a couple other things here and see if we can get your phone service working again quickly. Since you've done the reset button already, I'd recommend we do a hard reset of the phone modem by fully power cycling it. To do that - unplug the power and coax cables from the back of it, then remove the battery. Confirm that all the lights are off on the front of it, and then put it back together. Give it a couple minutes to reboot after that.
