Constant disconnects from Final Fantasy XIV, possible Shaw routing issue?

Hello, so ever since our modem got replaced with the CGNM-2250 model, everything else so far has been working fine, minus brief ping spikes here and there. I am using only the modem at the minute to connect to the internet. Just about every other application and game works fine, except for Final Fantasy XIV, which I am getting frequent disconnects from. I have a feeling it just might be a routing issue, is there any way for this to be fixed on you guys' end?

Seems like there's massive packet loss on that second to last hop there, could it be a problem with NTT?

Here's also my tracert results.

Is there anything I can do about this? Anyone I can contact? I have tried everything in an effort to solve this, including forwarding ports, changing DNS address to google's, changing internet explorer settings, checking my firewall etc.

 

Also, is this simply an issue with my modem, or is it just the path my connection takes? As I said, every other application and website works perfectly.
