Constant Disconnect/Timed Out?

What happens is that the connection would randomly be timed out/disconnect throughout the day, whenever this happens the modem-router status page would show that the WAN IP, WAN Gateway IP and DNS got changed to 0.0.0.0, and it may takes at least 2~5mins for the router to change the IP and DNS back to normal. This have been happening for at least a month.

 

Is there anyway to fix it? The modem/router I am using is SMC D3GN.
