I am also having the same issue. We did a self instal and I messaged before and someone responded and said it should be in a pamphlet. I did not get a pamphlet.
It may have been included in the package with the modem - I'm not sure if you picked it up from retail or had it mailed to you. If it wasn't included, then any customer service representative should be to confirm it for you. I will send you a private message so we can collect your account information to look it up.
